Position:  Care Coordinator-Seminole County

Description:  To Provide case management services to youth and families with mental health needs utilizing the wraparound model. This position is in partnership with Wraparound.

Responsibilities:

  •  
    • Maintain a caseload of 10-12 families with a minimum of 10 hours of service contact per month per family to include bi-weekly face-to-face contacts with the youth AND family at times and locations that are convenient to the family (including weekends and evening hours).  
    • Establish the initial contact with the family within 2 business days of referral/case assignment and meet with the youth AND family for an initial visit within 7 calendar days of referral. 
    • Exceptions will be documented in a progress note.  
    • Assemble a youth and family team within 14 calendar days of enrollment (first contact) by interviewing the family, identifying family members, natural supports, agency representatives and other significant persons. 
    • Services will be community based meaning Youth and Family Team meetings will occur in the family's home or community location agreed upon by the family.  
    • Assess the youth and family's strengths and needs and promote strategy development with assistance from the team members.  
    • Coordinate the meetings of the Youth and Family Team including setting the agenda, providing copies of the Youth and Family Care plan, contacting members to be invited, etc.  
    • Facilitate with the Team, the development of the initial Individualized Youth and Family Care Plan and Crisis Plan based on the youth and family's strengths, needs, and desires. 
    • The plan should reflect the best possible fit with the cultures, values and beliefs of the family.  
    • Complete the initial Youth and Family Care Plan within 30 calendar days. 
    • Subsequent updates are to be completed at a minimum, every 90 calendar days.  
    • Monitor the provision and quality of services through the Youth and Family Team, and modify the Plan per the guidelines.
    • Youth and Family Team meetings must be held at a minimum every 30 calendar days to review the progress.  
    • Provide oversight, leadership and development to a family partner from Wraparound, when a family partner is assigned to a specific case. 
    • The family partner must be included in all Youth and Family Team meetings and will have an active co-facilitation role. 
    • It is expected that family partners will eventually lead team meetings.
    • Assist the youth and family (including parents/guardians and siblings) in accessing mental health, social, educational, and other services and supports as identified by the Youth and Family Team.   
    • Ensure that the services being provided are responsive to the vision and needs identified in the Youth and Family Care Plan and that the family is experiencing voice, access and ownership of the plan and services.
    • Seek community resources and informal supports first with the assistance of the Youth and Family Team. 
    • The expectation is that by the second Youth and Family Care Plan meeting, the Youth and Family Team is comprised of a minimum of 50% of informal supports.  
    • Act as an advocate/ liaison for the child/family when services need to be sought/developed. 
    • Advocacy is included in numerous systems; dependency, delinquency, and school.
    • Provide or secure support and crisis/emergency services for the youth and family. 
    • This may be done through face-to-face contact, phone contact, or availability through an on-call system. 
    • The WS is expected to be available to their assigned families after hours and on weekends, unless prior arrangements have been made and approved by the Clinical Director.  
    • Be available 24/7, 1 week per month during an on-call rotation to respond to crisis situations or requests for assistance from youth and their families not assigned to the WS, but as a measure to provide coverage for families whose WS is unavailable due to vacation or illness.  
    • Collaborate with other necessary individuals the child/family may have contact with, i.e., Probation officers, Judges, Attorneys, Teachers, Physicians, etc. 
    • Provide or Arrange for transportation for his/her assigned youth to appointments, crisis/respite services, etc. if needed and with assistance from the Youth and Family Team.  
    • Utilize Wraparound documents (treatment plan, assessment, biopsychosocial, etc) and complete all necessary paperwork in an accurate and timely manner per Wraparound and Agency requirements, i.e. court letters, plans of care, progress notes, team meeting notes, consent forms, evaluation instruments, etc. 
    • Input all data into the Wraparound data system.
    • Participate in all required trainings, in-services, clinical supervision meetings, etc.and Quality Assurance/Improvement actvities.
    • Complete other duties as assigned by the Clinical Director, Lead Wraparound Specialist and/or Project Director (co-supervision model) and participates in bi-weekly supervisory meetings with the Wraparound  Management Team.    
    • Participate in and obtain family and youth participation in monthly National Federation of Family Meetings and periodic family and youth events.   
    • Complete required training courses within designated time frame.
  • Qualifications, Education, Experience:
    • Bachelor's Degree in social work or a human services related area of study from an accredited college or university and one year of related experience in human services with preference given for experience with strength based wraparound models.
    • Successful background screenings through local law enforcement, Florida Dept. of Law Enforcement, and FBI.
    • Bilingual Spanish or French-Creole preferred.
    • Valid drivers license and auto insurance, and successful motor vehicle driving record background check. (3 year driving history is checked.

Position:  Independent Living Coordinator Supervisor (Orange County)Description:  To oversee the daily operations of the Orange County IL program, ensuring services are provided with quality and in accordance with Federal, State and lead agency guidelines ensuring that children are prepared to live independently at age 18.

Responsibilities:

  •  
    • Ensure and assist all children to progress educationally and develop life skills to live independently.
    • Provide exemplary role model for staff and clients.
    • Assess need and insure timely access to emergency cash assistance (Aftercare funds) for eligible youth ages 18 to 23 to prevent homelessness.
    • Provide crisis counseling and be available to clients on an on call basis.
    • Pre-IL: ensure all youth are provided IL services as appropriate and within contractual timeframes.
    • Road to Independence:  Ensure clients meet eligibility requirements for RTI/TSS programs and maintain proper documentation in client files.
    • Schedule and provide regular client contact which includes home visits as needed.
    • Act as client advocate in developing Orange County resources for youth and young adults.
    • Timely documentation of all client contact in FSFN and maintenance of FSFN database and related reports.
    • Report all serious incidents to and maintain good communication with supervisor.
    • Act as program representative/advocate in interface with community partners.
    • Attend community meetings as representative of Intervention Services.
    • Provide training and education regarding Independent Living Services to child welfare family case managers; direct care staff and/ caregivers.

Position: Clinical Manager

Description:  To provide supervision and advice to primary clinicians regarding clinical and case management issues. To serve as Medicaid Treating Practitioner for designated sites.  

Responsibilities:

  •  
    • Providing effective supervision to assigned clinicians, in the form of group supervision 1x per month and on an individual basis as needed.
    • Providing effective supervision and training to Licensed Evaluators.
    • Responds to emergency and non-emergency calls from assigned clinicians.
    • Keep notes and supervision logs regarding content of consultation on client cases.
    • Reviews for appropriateness and signs, if appropriate, all treatment plans, reviews, psychosocials, behavioral programs, extension requests and discharge summaries completed by assigned clinicians.
    • Brings cases with liability issues or significant clinical concerns or conflicts to the attention of the Program Manager.
    • Turns in Supervision Log at least every 2 weeks, documenting date, start time, and amount of time spent in consultation with each clinician.
    • Effectively handling all complaints received that relate to clinical specific issues and subsequently address with respective clinicians.
    • Develop treatment team process to include as applicable client, caregiver, caseworker, psychiatrist, and clinician.
    • Perform clinical case consultations in the field and office settings Supervise assigned positions, including training, individualized performance planning and corrective action. 
    • Complete employee performance evaluations by the due date.
    • Monitor peer review process to ensure compliance and appropriate utilization of services Review and approve TBOS requests submitted by clinicians.

Position:  Independent Living Coordinator (Orange County)

Description:  Implement all aspects of the Independent Living program for eligible youth who are transitioning out of the Child Welfare System.

  • Responsibilities: 
    • EDUCATION:
    • Monitor academic performance of all youth assigned, coordinating with youth, caregiver and school personnel.
    • Enlist support of those involved in the progress toward and attainment of educational goal.
    • Provide educational support and training to caregivers to be advocates for the youth in their homes.
    • Train and assist caregivers in the implementation of live skills and normalcy plans.
    • LIFESKILLS & TRANSITION:  
    • Schedule and provide regular client contact which includes home visits.
    • Schedule and facilitate all Independent Living staffings at required intercals and time frames.
    • Provide individual life skills training per performance measure requirements.
    • Attend court, and complete JRSSR and/or IL status reports in a timely manner.
    • Maintain client files with all required independent living documentation to meet contract requirements and RTI/TSS eligibility.
    • Complete all RTI and TSS renewal applications in a timely manner.

    • Assemble new client charts according to Intervention Services, Inc. procedures.
    • File documents in client records in timely and correct manner.
    • Copy and mail client records when requested and with appropriate releases.
    • Follow HIPAA and Intervention Services’ policies regarding confidentiality of client records.
    • Communicating with Program Manager regarding liability issues or other significant events.
    • Provide assistance to the Psychiatric Clinic Assistant when needed to obtain prior authorizations for medications, set up appointment for psychiatric evaluation, follow up or reschedules with clients.

Description: Oversight of donor, funding and resource development activities in accordance with the ISI Development Plan. Program development and oversight of daily operations for new programs as assigned.

  • Responsibilities: 
    • Fundraising
    • Establishes short- and long-term goals for unrestricted funding sources and enlists support from members of institution staff.
    • Strategizes and orchestrates methods of approach to institutional donors.
    • Researches public and private grant sources (agencies, corporations, and foundations) to identify sources of restricted and unrestricted funding.
    • Works with program staff to develop
      funding-related projects.
    • Organizes individual donor campaigns (e.g. major
      donors, direct mail, and Board of Trustees).

Position:  Psychiatric Clinic and Office Assistant (Maitland Office)

  • Description: Work closely with the Referral Coordinator, Psychiatric Clinic Assistant and Program Manager to respond to client's needs and to coordinate care.  Coordinate and facilitate support of tasks related to the phone system, researching records, customer service, and general office needs.
  • Responsibilities: 
    • Answer telephones in a professional manner, route calls and
      deliver messages.
    • Contact clients who have left phone messages within 24 hours.
    • Distribution of mail and faxes to all locations: Exchange,
      Maitland, Osceola and Brevard.
    • Provide support with tasks related to the coordination of care for clients (phone calls to primary care physician (PCP), fax information, requesting of copies of records from other agencies). Send PCP forms.
    • Set up appointment for psychiatric evaluation, follow up or
      reschedules, and Intake appointments for license evaluator.
    • Call clients to remind and confirm appointments for the
      Psychiatric Clinic and office license evaluator.
    • Call no-show clients of Psychiatric Clinic to reschedule
      appointments.
    • Send letter for the no-show clients of the license evaluator.
    • Help making and inputting referrals and check Medicaid for
      clients as needed.
    • Enter new and update existing client information into the
      database making sure that it's up to date and correct.
    • Provide support to the general office maintenance. 
    • Interact with clients, caregivers, outside agencies, and other
      staff in a professional and courteous manner.

Position: Psychiatrist

  • Description: To provide psychiatric evaluation, medication management, ARNP and Peers supervision and leadership for the Outpatient and Clinic services for multiple sights.
  • Responsibilities
    • Abide by the ethics, policies and procedures, and professional standards of Intervention Services, Inc.
    • Ensure compliance with regulatory, accreditation and licensing standards as well as agency policies and procedures. ­
    • Ensure the provision of high quality and timely access to client services. ­
    • Maintain regular communication with the Program Director, including potential liability issues or significant incidents that put the agency or the clients at risk.­
    • Provide effective and efficient psychiatric evaluations, explanations of psychiatric evaluations to family members and medication management.
    • Provide peer reviews for cases that have been seen by other doctors and nurses at ISI.
    • Ensure that prescriptions for medications will follow the formulary lists provided by the funders whenever possible. Written prescriptions will follow state and federal rules and regulations.
    • Ensure the supervision of ARNP and Other MD’s and be sure that they are following ISI procedures and that the documentation is in compliance with the Medicaid Community Mental Health Manual requirements for progress notes.
    • Ensure that every office will comply with a treatment plan for cases.
    • Being available for emergency consultations and respond to these emergency calls in a timely manner.  
    • Identify complex clients in order to respond appropriately to the client’s medical needs.
    • Ensure appropriate communication between the staff regarding the PCP and the coordination with other professionals as needed.
  •  Qualifications, Education, Experience:
    • MD, Florida psychiatric license and at least two years post licensure experience providing direct services in the clinical specialty area

Position:  Outpatient Services Clinician (Maitland, Rockledge and Kissimmee Offices)

  • Description:To provide in-home and in-school clinical services for children, adolescents, and their families.
  • Responsibilities: 
    • Work independently to provide clinically effective services to clients, including in home-based and in school setting, based on the needs of the case.
    • Develop and maintain a professional working relationship with various agency professionals, schools, and other community resources.
    • Provide consistent clinical services to clients assigned, following up to either resume services or discharge following missed sessions.
    • Contact assigned referrals within 48 hours upon receipt of referral to schedule intake sessions.
    • Schedule intake sessions within 10 days of assignment.
    • Adhere to and maintain a predetermined minimum amount of monthly billable services as outlined in the Rate Agreement.
    • Coordinate services to avoid billing or clinical conflicts if there are multiple providers on the case.
    • Communicate with the referral source, especially to notify him/her of the intake, discharge, or "dead file".
    • Complete client documentation, including required clinical paperwork, and submit originals in a timely manner.
    • Document missed sessions and weeks without service to be placed in the client record.
    • Obtain approval to extend cases beyond 6 months prior to the expiration date.
    • Notify the Licensed Evaluator within 5 days of referral assignment that a face-to-face evaluation needs to be done.
    • If client is under 5 years of age, clinician will ensure that they will have a Licensed Evaluator present with them at intake.
    • Participate in at least monthly supervision with the assigned Clinical Supervisor.
    • Follow HIPAA rules and ISI Policies & Procedures regarding confidentiality of client information.
    • Follow state mandated abuse & neglect reporting procedures and ISI Policies & Procedures regarding abuse/neglect of children, disabled individuals, or elderly individuals.
    • Submit Incident Reports for sentinel events, as specified in ISI Policies & Procedures.
    • Maintain a confidential voice mail phone number to be checked on a daily basis and ISI email to be checked on an at least a twice weekly basis.

  • Qualifications, Education, Experience:
    • Requires Masters Degree in social work or a human services related field from an accredited college or university.

Position:  Bilingual Outpatient Services Clinician (Kissimmee Office) 

  • Description:To provide in-home and in-school clinical services for children, adolescents, and their families.
  • Responsibilities: 
    • Work independently to provide clinically effective services to clients, including in home-based and in school setting, based on the needs of the case.
    • Develop and maintain a professional working relationship with various agency professionals, schools, and other community resources.
    • Provide consistent clinical services to clients assigned, following up to either resume services or discharge following missed sessions.
    • Contact assigned referrals within 48 hours upon receipt of referral to schedule intake sessions.
    • Schedule intake sessions within 10 days of assignment.
    • Adhere to and maintain a predetermined minimum amount of monthly billable services as outlined in the Rate Agreement.
    • Coordinate services to avoid billing or clinical conflicts if there are multiple providers on the case.
    • Communicate with the referral source, especially to notify him/her of the intake, discharge, or "dead file".
    • Complete client documentation, including required clinical paperwork, and submit originals in a timely manner.
    • Document missed sessions and weeks without service to be placed in the client record.
    • Obtain approval to extend cases beyond 6 months prior to the expiration date.
    • Notify the Licensed Evaluator within 5 days of referral assignment that a face-to-face evaluation needs to be done.
    • If client is under 5 years of age, clinician will ensure that they will have a Licensed Evaluator present with them at intake.
    • Participate in at least monthly supervision with the assigned Clinical Supervisor.
    • Follow HIPAA rules and ISI Policies & Procedures regarding confidentiality of client information.
    • Follow state mandated abuse & neglect reporting procedures and ISI Policies & Procedures regarding abuse/neglect of children, disabled individuals, or elderly individuals.
    • Submit Incident Reports for sentinel events, as specified in ISI Policies & Procedures.
    • Maintain a confidential voice mail phone number to be checked on a daily basis and ISI email to be checked on an at least a twice weekly basis.
    • Follow all other ISI Policies & Procedures contained in the Outpatient Services Training Manual.
  • Qualifications, Education, Experience:
    • Requires Masters Degree in social work or a human services related field from an accredited college or university.
    • Prefer Licensed or Certified in the state of Florida as an LCSW, LMHC, LMFT BCABA or BCBA; OR
    • ­Ph.D. in Psychology from an accredited college or university.
